New World Screwworm: An Evolving Animal Health Crisis — Clinical Guidance, Causes, Prevention, Treatment, and Supportive Products

New World screwworm (NWS), caused by the parasitic fly Cochliomyia hominivorax, has re-emerged as a major animal health threat in North America. Eradicated from the United States in the 1960s through the sterile insect technique (SIT), the pest advanced northward through Central America (significant outbreaks from 2023) and Mexico (detected November 2024), reaching the U.S. in June 2026.

As of late July 2026, USDA-APHIS confirmed approximately 37 domestically acquired animal cases, primarily in Texas and New Mexico, affecting cattle, sheep, goats, and at least one dog. No locally acquired human cases have been reported in the U.S., and public risk remains very low, but the economic threat to livestock, wildlife, and companion animals is substantial.

NWS larvae feed exclusively on living tissue of warm-blooded animals. Untreated infestations can kill animals within 7–14 days. The U.S. response includes aggressive surveillance, quarantines, movement restrictions, sterile fly releases, and expanded access to treatments via FDA emergency use authorizations (EUAs).

What Causes Screwworm Infestation?

Adult female NWS flies are attracted to open wounds, mucous membranes, or orifices (eyes, nose, ears, mouth, genitalia, umbilicus of newborns). Even minor injuries — tick bites, scratches, surgical sites, shearing cuts, or birth-related wounds — can serve as egg-laying sites.

A single female lays 200–400 eggs (up to ~3,000 over her lifespan). Eggs hatch in 10–21 hours. Larvae use sharp mouth hooks and "screw" into healthy living tissue, feeding for 5–7 days. They enlarge the wound, produce a characteristic foul odor, and attract additional flies. The full life cycle can complete in as little as 21 days in warm climates.

Clinical Signs and Diagnosis

Early detection is critical. Signs include:

  • Rapidly expanding, painful, foul-smelling wounds that fail to heal
  • Visible larvae oriented head-down with posterior spiracles exposed
  • Swelling, bleeding, discharge, or secondary bacterial infection
  • Lethargy, reduced appetite, and progressive tissue destruction
  • In severe cases, deep tunneling into muscle or vital structures

Infestations can occur in any warm-blooded animal. Newborns (umbilicus), animals with recent wounds or surgery, and those in endemic areas are at highest risk.

Diagnosis requires larval identification submitted to official laboratories. Suspected cases must be reported immediately to state animal health officials and USDA-APHIS. Do not discard live larvae outdoors — they can pupate and establish local populations.

Clinical Guidance for Veterinarians and Animal Owners

  1. Immediate reporting — Contact state/federal authorities before or concurrent with treatment
  2. Personal protective equipment — Wear gloves; handle larvae carefully
  3. Larval removal — Physically extract all visible eggs and larvae (forceps preferred). Apply an approved insecticide to the wound first. Collect specimens for confirmation
  4. Wound management — Thoroughly clean, debride necrotic tissue if needed, and cover when possible. Monitor for secondary infection
  5. Systemic and topical antiparasitics — Use FDA-authorized products. Re-examine wounds at 24–48 hours
  6. Supportive care — Analgesics, antimicrobials for secondary bacterial infection, fluid therapy, and nutritional support as indicated. Isolate affected animals until wounds heal
  7. Herd/group management — Quarantine, treat at-risk animals prophylactically, and implement fly control

Prevention Strategies

  • Wound protection — Clean and cover all wounds immediately, no matter how small. Treat umbilical cords of newborns. Minimize elective wounding procedures during high-risk periods
  • Fly and external parasite control — Use approved insecticides, ear tags, pour-ons, sprays, or dust bags containing permethrin or organophosphates. Control ticks and other wound-causing parasites
  • Husbandry — Inspect animals frequently, especially those with wounds or recent procedures. Remove sharp objects from pens and transport vehicles
  • Travel and movement controls — Inspect animals and vehicles after travel to endemic areas. Follow quarantine and livestock movement restrictions
